Important Information

Jones Health Center and Physicals: Action Items

Where we are:
Jones Health Center is located in the center of campus, not far from the Bell Tower.

Who we are:
Most days the center is staffed with a family nurse practitioner (Rebecca Nelson) and a registered nurse (Gina Taylor) who work together to keep students healthy and safe during the school day.

We are able to dispense some over-the-counter medications to students during the school day as needed. An example: ibuprofen/acetaminophen for a student’s headache or cough drops for a cough. In order for us to give any over-the-counter medications, we must have an annual medication permission form on file, signed by both the parents and the physician.

Forms and Requirements:
There are forms we require and there are forms the state requires. While we are an independent school we still must follow N.C. guidelines with regard to immunization and physical requirements. The State of N.C. allows a 30-day grace period from the first day a student is enrolled, and we extend the same grace for our forms/requirements (with the exception of Athletics, whose forms which must be completed before an student can try out for a team).

Please be aware if required forms are not completed within 30 days of admission, access to school/school activities will may be limited.

N.C. requirements for new students:

  • Recent Physical (needs to be within the past 13 months)
  • Current Immunization Record

Forms required by Ravenscroft:

  • Vital Health Record
  • Consent for Medical Treatment
  • Consent for Off-Campus Activities

For our international families or children not previously vaccinated in N.C.: you can visit http://www.immunize.nc.gov/ to review requirements, or just give us a call and we can clarify N.C. immunization requirements based on grade level.

Don’t wait until the last day, as the logistics of logging into the system, and getting physician-signed physicals and immunization records can be time consuming.

Magnus login instructions/link:

  1. Log onto the Magnus health website (use the same username and password as Veracross)
    https://secure.magnushealthportal.com/vclogin/login.jsf?schoolID=248995
  2. Read through and answer the conditional questions
  3. Complete the vital health record (yes/no questions regarding your child’s overall health)
  4. Complete the online signature in Consent for Medical Treatment section and Consent for Off-Campus Activities
  5. You will be able to print any needed forms from Magnus, and once completed be able to scan/upload the forms directly into Magnus

Additional forms; some require both physician and parent signatures:

  • Over-the-counter Medication Permission Form, required if you want your child to get over-the-counter medications if needed during the school day (for example, Tylenol for a headache)
  • Prescription Medication Permission Form, required if your child needs medications administered DAILY at school
  • Asthma Action Plan, required if your child has asthma
  • Migraine Action Plan, required if your child has migraines
  • Allergy Action Plan, required if your child has severe allergies to food or insects
  • Diabetes Action Plan, required if your child has diabetes
  • Seizure Action Plan , required if your child has seizures

Additional forms are required for participation in Ravenscroft Athletics.

The scan-upload process may sound daunting. The easiest way is to snap a picture using a smartphone, and the form can be immediately uploaded. However if you run into any difficulty during this process, the Parent Helpline at Magnus is 1-877-461-6831 (and I have found them to be MOST helpful) or just give me a call 919-847-0900 (x2239 or x2845).
